We would like to extend a warm welcome to you as a new patient. In visiting our office, you'll find that we are dedicated to providing the most comfortable, high-quality periodontal care possible.

This brochure has been developed so that we can answer some of the questions you may have about initial periodontal treatment. We appreciate that taking the first step is not always easy. However, we are confident that you will find our relationship to be beneficial to you.

Establishing and Protecting Oral Health

You may have been referred to our office for one of many reasons related to your oral health. Based on our diagnosis, in consultation with your general dentist we will develop a treatment plan specifically for you.

Periodontal disease is similar to other chronic diseases, such as diabetes. The key to control is early diagnosis and prompt ongoing treatment. Once treated for periodontal disease, periodontal maintenance procedures will enable you to gain control of the disease and increase your chance of keeping your teeth. Ongoing periodontal maintenance will ensure for your optimum oral health and the highest quality care possible.

On your First Visit

Your initial visit will take approximately an hour. When you arrive, you will be asked to complete a medical/dental questionnaire for us to keep here on file in the office. Please arrive 15 minutes prior to the appointed time to complete this form. If you must reschedule this reserved time, please advise us at least 48 hours in advance otherwise a cancellation fee will be applied.

During the initial visit, we will provide a thorough oral examination, x-rays if needed, and Dr. Mirbod will meet with you personally to discuss the course of treatment best suited to your needs.

Financial Arrangements / Dental Insurance

Please bring along insurance information for use in completing the personal information section. Our staff will work with you to file insurance claims and discuss financial arrangements if necessary.

For the first appointment we do not accept direct assignment to your insurance company. In other words, for this appointment you will be asked to pay directly and then get reimbursed by your insurance company accordingly. The fee for the first visit will range from $86 to $136 plus the cost of x-rays (to a maximum of $104).
